Grant Eligibility & Distribution

  • Applicant must be a family caregiver or legal guardian of an individual diagnosed with Alzheimer’s or dementia

  • Demonstrated financial need

  • Funds requested must be for Alzheimer’s-related care or support

  • Applicant resides within the Foundation’s designated service area

Grant Application Process

If you are interested in applying for a grant, please complete the application. If you meet the specific qualification criteria, your application will be reviewed monthly by the Hall of Hope Family Foundation Board.
